This document is entitled “Shipley-Hartford Retreat Scale for Measuring Intellectual Impairment.” It provides directions and a scoring key. It was developed by the Neuro-Psychiatric Institute of the Hartford Retreat. It was copyrighted in 1940 by the same institution. The document contains an insert that appears to be a version of the test (copyrighted 1939).
